Part Two - Submitting Your Application
DBA/Operating Name
If you will be doing business under a name not listed in your organizational documents then you will need to list the name you will use. The DBA/Operating Name must be certified with your state or county if applicable.
Reason for the Application
Are you starting a new business, wanting to open another new location, continuing your current business but it will be located in a different state than what originally approved for, purchasing an existing TTB approved business, changing your business structure, such as going from a limited liability company to an corportation.
Commodity Operation Information
In this section questions will be asked that pertains to the type of application you are applying for. For example, if you are applying as a distilled spirits plant you will be asked to describe the size and use of each building and where doors are located.
