Part Two - Submitting Your Application
Background:
You will begin the process by going through the “New Applications Wizard”. You will be asked some questions to confirm what application(s) you are applying for and then the Wizard will create what is called your Application Package.
When applying to start a new TTB-regulated business, your application package will contain both an application for original entity and one or more new commodity operations applications (one for each permit, registration, or notice you need). The application for original entity will establish the Permits Online entity record for your business based on your Employer Identification Number (EIN). Think of commodity operations as a permit, registration, or notice, for example, a brewers notice, distilled spirits plant registration, or wholesaler permit. All of the information related to each TTB-regulated operation conducted by the business entity is contained in a commodity operations application.
For most application types, your application package may also include one or more Personnel Questionnaires. The Personnel Questionnaire, or PQ, (formerly called Owner Officer Information Application, or OOI) is how TTB gathers background information about owners, officers, and others associated with TTB-regulated businesses. We use the PQ to determine the eligibility, suitability, and/or qualifications of an applicant.
Example of the information you will see captured within each section
- Application for Original Entity - Information regarding owners, officers, members, managers, directors, and others that are part of the business. Business name, signing authority, power of attorney, and any trade names for labeling and/or bottling on account for purposes.
- Application for Commodity Operations – Location-specific information, such as: operations being conducted, "doing business as" (dba) names, and (if applicable) bonds, variances, and information on non-contiguous locations.
- Personnel Questionnaire (PQ) – Personal information pertaining to the persons associated with the business. More details on the PQ will be in the PQ section of this tutorial.
