Personnel Questionnaire
A Personnel Questionnaire (PQ) will need to be completed for certain individuals associated with your business.
A PQ is required for the following:
- Sole proprietor
- Partners
- Officers
- Directors
- Trustees
- Members, managers, and/or managing members
- Stockholders holding 10% or more of the shares in a corporation
- Interestholders holding 10% or more of the shares in a limited liability company
The following do not need to complete a PQ:
- Tax Free Alcohol Users
- Specially Denatured Users and Dealers
- United States Government Agencies
