Entity Amendment Wizard	The Entity Amendment Wizard guides you by asking a series of questions, and depending on your answers creates an application package, which is a list of the applications you're required to submit. Everyone who applies for an amended entity must complete the questions in the Entity Amendment Wizard.
	File this application to report:
Amended Application for Entity Information	Change controlling ownership Change any partner or general partner Change any stockholder or members holding ownership of 10 percent or more Change corporate officer, directors, or any titled positions Add, revoke, or change signing authority Add, revoke, or change power of attorney Change the legal business name
Amended Application for Trade Name	New labeling or bottling on account for trade names Removal of labeling or bottling on account for trade names
Amended Application for Termination of Business	Termination of all TTB-regulated operations under an EIN
Personnel Questionnaire	The Personnel Questionnaire is how TTB gathers background information about owners, officers, and others associated with TTB-regulated businesses. You may need to complete PQs if you are adding new owners, officers, or certain other people to your business entity.
